Then print out the form, sign it, and mail OR fax (not both) the hardcopy to TMRS. If you print the form out and fill it in by hand, please type or use black ink. Also, please do not use highlighter on forms. You will need Adobe Acrobat Reader to read our forms.

Reminder to Use Current TMRS Forms
The following forms were recently revised, and TMRS no longer accepts the old versions of these forms:

NOTE: Everyone — cities and members — needs to use the new forms. If old forms are submitted, TMRS will return them and request resubmission of the new forms. Please help get this information out to your members and also replace your old forms with new ones. The revision date of forms can be found at the bottom left corner of each form, for example: TMRS - 080E • Revised 7-2010.
